The question

A nurse is caring for an agitated older adult client who has been attempting to pull out their IV line and climb out of bed. The nurse is considering the use of physical restraints. Which action should the nurse take first before applying restraints?

  1. Explore and implement the least restrictive, non-restraint alternatives to manage the behavior
  2. Document the behavior in the medical record and wait for the primary health care provider to initiate a restraint order
  3. Notify the family that restraints will be applied as a routine fall-prevention measure
  4. Apply soft wrist restraints to both wrists to prevent the client from removing the IV
