NCLEX-RN® question of the day

The question

Marisol, a 58-year-old woman with type 2 diabetes and a 30-pack-year smoking history, comes to the outpatient clinic reporting cramping pain in her right calf that starts after walking about two blocks and stops when she rests. She works a warehouse job that requires her to be on her feet most of the shift. Her feet are cool to the touch and pedal pulses are diminished bilaterally, with the right foot slightly paler than the left when elevated. The nurse recognizes this pattern of pain is caused by which underlying mechanism?

  1. Venous valve incompetence causing pooling of blood and increased venous pressure in the lower leg
  2. Arterial narrowing that limits blood flow to exercising muscle, creating a temporary oxygen supply and demand mismatch
  3. Nerve root compression from lumbar spine disease reproducing pain with ambulation
  4. An acute clot obstructing a deep vein and triggering local inflammation
