The question

A 58-year-old male is admitted to the ICU following emergency abdominal surgery. On post-operative day 2, his urine output has been 15 mL/hr for the past 3 hours, his serum creatinine has risen from a baseline of 0.9 mg/dL to 2.4 mg/dL, and his blood pressure is 88/54 mmHg. The nurse notes his central venous pressure (CVP) is 2 mmHg and his mucous membranes are dry. Which intervention should the nurse anticipate implementing first?

  1. Prepare the client for emergent hemodialysis
  2. Administer a prescribed dose of furosemide IV to increase urine output
  3. Administer a prescribed IV fluid bolus to restore intravascular volume
  4. Insert a urinary catheter to obtain an accurate urine output measurement
