The question

In the emergency department, a nurse is assessing a 7-year-old child brought in by a parent after falling from a playground structure. The child is alert, crying, and holding the left arm still, and the parent reports the child landed on an outstretched hand. What would the nurse expect to find if the child has sustained a closed fracture of the forearm?

  1. An open wound with visible bone protruding through the skin.
  2. Localized swelling, pain, and deformity of the affected extremity without an open wound.
  3. Full, pain-free range of motion of the affected limb.
  4. Complete absence of pain due to nerve transection at the fracture site.
