The question

On a busy inpatient psychiatric unit, the charge nurse is assigning tasks for the shift. A client with generalized anxiety disorder is scheduled for a routine assessment, another client newly started on an antidepressant needs a check on adherence, and a third client requires assistance with activities of daily living. An unlicensed assistive personnel (UAP) and a licensed practical nurse (LPN) are available to help. Which task should the charge nurse delegate to the UAP?

  1. Assessing the client with generalized anxiety disorder for escalating symptoms
  2. Teaching the client about potential antidepressant side effects
  3. Evaluating the new antidepressant for therapeutic effectiveness
  4. Assisting the client with activities of daily living
