- K — Knowledge gap: never learned or forgot the content
- A — Application gap: knew the content, could not apply it to the scenario
- R — Read wrong: misread the stem, missed a qualifier like "first" or "most"
- C — Changed answer: had it right, talked myself out of it
- T — Time or test anxiety: rushed, blanked, or panicked
