The question

A nurse is preparing to assess a patient admitted with a non-healing leg wound. Before beginning the physical examination, the nurse conducts a health history interview. Which question would be most appropriate to ask first when gathering information about the patient's wound?

  1. Have you had any surgeries in the past five years?
  2. What medications are you currently taking for pain?
  3. What is your current diet like on a daily basis?
  4. Can you tell me what has been going on with your wound and when it first appeared?
