The question

A 32-year-old woman is admitted to the medical-surgical unit for evaluation of persistent fatigue and insomnia following a recent diagnosis of a chronic autoimmune infection requiring long-term antiviral therapy. During data collection, the LPN notes the client is tearful, with a flat affect, and oriented to person and place only. She states, "I haven't slept in days. My partner left after my diagnosis, and my family stopped calling. Sometimes I think everyone would be better off without me." She adds that she identifies as bisexual and prefers her chosen name be used, and that she relies on prayer during stressful times but does not want a chaplain visit at this time. Which statement by the client requires the LPN to take immediate action?

  1. "My partner left after my diagnosis, and my family stopped calling."
  2. "Sometimes I think everyone would be better off without me."
  3. "I identify as bisexual and prefer that my chosen name be used."
  4. "I haven't slept in days and I feel exhausted all the time."
