The question

An LPN is caring for a 78-year-old man on a medical-surgical unit who is on contact precautions for a wound infection. He is scheduled for his annual influenza vaccine and is due for the pneumococcal (PPSV23) vaccine per his immunization record. His history includes type 2 diabetes and a penicillin allergy; he takes metformin daily. He reports feeling "a little tired" but denies fever. Vital signs are temperature 37.2°C (99°F), heart rate 88/min, respirations 18/min, blood pressure 132/78 mm Hg, and oxygen saturation 97% on room air. Which action should the nurse take?

  1. Tell the client to stay in bed and delay vaccines until his fatigue resolves completely.
  2. Administer the influenza and pneumococcal vaccines as scheduled since findings show no true contraindication.
  3. Advise the client to research vaccine safety on an online health forum before deciding.
  4. Withhold both vaccines today because the client is on contact precautions for infection.
