The question

A 32-year-old woman with type 1 diabetes mellitus is seen in the outpatient clinic for a follow-up visit after adjusting her insulin regimen. Her baseline fasting glucose two weeks ago was 98 mg/dL. Today's point-of-care fasting glucose is 45 mg/dL. Vital signs are BP 108/68 mmHg, HR 96 beats/min, RR 18 breaths/min, SpO2 98% on room air, temperature 98.4°F (36.9°C). The client reports feeling shaky and slightly confused. The LPN/VN obtained the fingerstick glucose as part of the visit. What should the LPN/VN do next?

  1. Report the glucose value and client symptoms to the RN or provider immediately.
  2. Give the client oral glucose tablets and adjust her insulin dose independently.
  3. Document the glucose result and shakiness in the chart and continue the visit without notifying the RN or health care provider
  4. Proceed with the scheduled insulin teaching session as planned.
