The question

An LPN visits an 82-year-old woman receiving continuous gastrostomy feeding at 60 mL/hour for dysphagia after a stroke. The tube was placed 3 weeks ago. Her daughter has paused the pump for care. The client has been lying flat because her daughter cannot reposition her alone. The LPN also notes new sacral redness and moisture from urinary incontinence. Before resuming the feeding, which action is best?

  1. Chart the redness and postpone discussing positioning difficulties until the next visit.
  2. Elevate her upper body with assistance and complete the prescribed feeding and skin-care safety measures.
  3. Resume the pump while she remains flat because tube feeding bypasses the mouth.
  4. Independently replace the prescribed formula with a thinner formula to reduce aspiration risk.
