The question

An 82-year-old woman recovering from hip-fracture repair also has residual hemiparesis from a prior stroke. Her current transfer plan requires two-person assistance and a gait belt. She can wash her face and brush her teeth with her unaffected hand. Today she becomes newly unsteady while sitting at the bedside. What should the LPN do first?

  1. Help her into a safe supported position and report the new unsteadiness to the RN before attempting transfer.
  2. Complete her face washing and toothbrushing for her, then continue with the planned transfer.
  3. Have one staff member transfer her alone now to minimize time at the bedside.
  4. Proceed with the scheduled two-person transfer and mention the unsteadiness at shift handoff.
