A 32-year-old client, G2P1, at 34 weeks is seen at a skilled nursing facility after hospitalization for hyperemesis and dehydration. Her baseline BP was 118/74 mmHg. Today BP is 152/98 mmHg; she reports a severe headache since morning and says the baby is moving less than usual. Fetal heart rate is 138/min and no contractions are noted. She asks whether the headache is from missing a meal. What should the LPN do first?
- Reassure her that missing a meal explains the headache and offer a snack.
- Immediately report the headache, BP change and reduced movement to the RN or provider and arrange urgent obstetric evaluation.
- Independently establish a new treatment plan for the presumed pregnancy complication before contacting the RN or provider.
- Document the findings and continue routine hydration and nutrition teaching.
