The question

A 54-year-old female resident in a long-term care facility has a stage III pressure injury on her sacrum and is scheduled for a bed-to-wheelchair transfer for a dressing change in the treatment room. The resident cannot bear weight on either leg and requires total assistance to move. A second nursing assistant and a mechanical floor lift are available on the unit. The wheelchair is positioned at the bedside on tile flooring, and the bed is currently unlocked with the side rail raised. Which action should the licensed practical nurse take first?
