The question

A licensed practical nurse (LPN) is making a home health visit to a 68-year-old woman recovering from a hip fracture. She has a weight-bearing-as-tolerated order on the right leg and uses a new front-wheeled walker for ambulation. Vital signs are temperature 98.4°F, pulse 78, respirations 16, blood pressure 128/74, and she reports no dizziness. The client is on peritoneal dialysis for chronic kidney disease and spends most of the day sitting in a recliner. Her sacral area is reddened but the skin remains intact, and her Braden Scale score is 14, indicating moderate risk. The client's daughter states she "just likes to keep her still so she doesn't fall." What should the LPN do to address this client's mobility and skin needs?

  1. Dress and transfer the client completely to conserve her energy for the day.
